Growing in GRACE II | Springhill, FL

Growing in GRACE II is open to the public for all hours and days of normal operation (Monday – Friday; 6:00 am – 6:00 pm).

Drop-in care is available on an open slot basis only with the consent of the Director.

Children (2 years – 5 years) may be enrolled throughout the year. The Center will not exceed our licensing capacity at any time.

Children will be accepted for enrollment at the Center on a first-come, first-served basis with availability of open slots taking precedence. Once the Center is at full capacity, children will be put on a waiting list and as slots open the Director will call parents in the order in which they were put on the list, according to scheduling and available slots open.

REGISTRATION PROCEDURE

Complete and submit these forms at least TWO weeks before your child’s first day of attendance:

A. Read the Parent Handbook to ensure you understand the policies and procedures of Growing in GRACE II.

B. Complete and submit the following forms at least TWO weeks before your child’s first day of attendance:

a. Application for Enrollment (HERE)
b. Copy of Birth Certificate or other proof of age (Health Certificate with D.O.B serves as proof of age)
c. Agreement of the Tuition and Fees (HERE) (2-3-year-olds, VPK and/or before or after care)
d. Florida Certificate of Immunization– Immunizations must be documented on DH Form 680 and must be signed by a practicing physician in the State of Florida. DH Form 680 may be obtained from a private physician or the local county Health Department, it may be possible to retrieve it online HERE. Must be kept current to the issue and expiration date and be received no longer than 30 days after the first day of enrollment.
e. Student Health Examination – The Student Health Examination must be documented on DH Form 3040 (FOUND HERE) and must be completed by a person given statutory authority to perform health examinations. DH Form 3040 may be obtained from a private physician or the local county Health Department. The examination is valid for 2 years from the date it was received. However, upon admission the examination must have been received within the past year and no later than 30 day from the first day of enrollment. Parents, please do not forget to fill out the backside of the physical form.
